Sacrament of Baptism
Parents should complete this form to start the process of having your child Baptized in the Catholic Church.
Parents & Godparents should sign up to attend a class. Please choose one of the options below:
Please Note that Baptisms will not be scheduled
until all the paperwork has been received.
Baptisms are routinely scheduled, as follows:
- The 2nd Sunday of the month in English after the 10AM Mass at Holy Trinity
- The 3rd Sunday of the month in Spanish at 12:30PM (prior to the Spanish Mass) at Holy Trinity
- The 4th Sunday of the month in English after the 11AM Mass at Our Lady Star of the Sea
Requirements:
Parents:
- Must be registered in the Parish for a minimum of 90 days.
- Must complete the Baptismal registration form
- an official copy the child's birth certificate has been sent to the parish
- parents will need to submit their Sacrament Certificates, Baptism, 1st Eucharist, Confirmation and Marriage if applicable.
- One parent must be a practicing Catholic, both parents need to agree to the child(ren) to be baptized.
- Complete Baptismal Preparation Class
Godparents:
- Must be at least 16 years of age
- Must be Confirmed in the Catholic Church, Byzantine Church or other Christian Faith recognized by the Holy See.
- If you are having two Godparents, one must be male, the other female
- Completed a Baptismal Preparation Class. If you have taken a class within the last two years, supply the completion certificate.
- A letter in Good Standing from their Parish's Pastor
IMPEDIMENTS TO BEING A GODPARENT INCLUDE:
- a. Being a non-Catholic
- b. Causal Church attendance
- c. Not Confirmed in the Catholic Church
- d. Failure to register and/or support home parish
- e. If married, you were not married in the Catholic Church
- f. Engaging in any practice in conflict with the Church teachings on faith and morals
